Application Process & Required Documents

- ADN Multi Criteria Screening Worksheet:Unofficial document for student/counselor reference use only. Do not submit this form with your ADN Application.
Apply to the traditional ADN program or the LVN to RN Advanced Placement program in 2 easy steps!
Step 1:
- All students must complete the ADN Multi Criteria Evaluation Request Form. (The evaluation request form will only be processed during the following evaluation period: August 5th, 2024, 8:00 a.m. through September 20th, 2024, by 4:00 p.m.
You will be prompted to log into your Chaffey College Portal and routed to the form once you click on the form link below.
Step 2:
Prepare supplemental application documents: Please use the following downloadable supplemental documents for consideration of extra points to be submitted with your ADN application (if applicable). * LVN to RN Advanced Placement applicants do not need to complete these forms for their applications.
- Recent work/volunteer experience verification form
- Demonstrated proficiency in a language other than English verification form
- Life experience/special circumstances area
